Intensive Case Management Access Coordination (IntAC)

This is a centralized process and single point of access to mental health intensive case management services provided by 4 agencies for residents of Hamilton.

Four agencies are:

  • Canadian Mental Health Association (CMHA)
  • Community Mental Health Promotion Program (CMHPP) - City of Hamilton
  • Hamilton Mental Health Outreach (HMHO)
  • Hamilton Program for Schizophrenia (HPS)

Eligibility

To be eligible for this service, the person referred needs to be:

  • Living in Hamilton
  • 16 years of age or older
  • Diagnosed with a serious and persistent mental illness (Axis 1)
  • Is under the care of a physician (some services require a Hamilton physician)
  • Willing and able to participate in working on goals, and requires to support to maintain wellness in the community
