Referrals for Children's Mental Health services and the Child and Youth Autism Programs are made through the centralized intake process (Contact Hamilton) at 905-570-8888. For more information on referrals to specific programs, please visit: woodview.ca/programs/"

Referrals for the Adult ASD Programs and Services are made through the DSO (Developmental Services Ontario) local office 1-877-376-4674.

Families seeking Passport services may contact Woodview directly at 905-527-9771 ext 179.

Eligibility

Age 5 years to 18 years for Children's Mental Health programs; 7-18 for Children's and Youth ASD programs in Hamilton (younger ages may access services in Halton); 18+ for Adults with ASD

Application

Hamilton: All referrals for the Child and Youth Autism Programs are made through the centralized intake process (Contact Hamilton) at (905) 570-8888. * Referrals for day treatment are made directly to Mountain or Delta Programs Adult ASD referrals are made through the DSO. Families with Passport funding may contact directly.

* Brant: All referrals are made through the centralized intake process (Contact Brant) at (519) 758-8228. * All clients seeking service must call Contact Brant directly. Exceptions include referrals for the TAPP-C (The Arson Prevention Program for Children) which are made directly by the Fire Departments of Brantford and Brant County, the Youth Justice Program which are made directly by Probation Officers, and Youth Justice Court-Ordered Assessments which are made directly by the court.

* Halton: Requests for the Halton Intensive Program are directed to the Program Manager, but will also be reviewed by Connections Halton to determine priority and need for Intensive service.

* Applicants should call the program directly.* All other referrals for service are made directly to the program. For ASD Ontario Autism Program services contact ErinoakKids. Adult ASD Passport and Fee for Service call 905-527-9771 ext 179. For ASD Woodview Learning Centre private school services call 905-689-4727 ext 100.
